If a child must take any medication during school hours, the school will require:

1. A written request from the parent or guardian.
2. The written order of the prescribing physician, indicating the type, purpose and dosage requirements of the medication.
3. All medication is to be dropped off and picked up by the parents or guardian at the beginning and end of the prescription period.
4. All medication must be in the original container.  Ask your pharmacist for a double label for school time doses.
5. Medication Forms are available in each building's School Health Office.

School personnel, under the direction of a school nurse-teacher or registered nurse, are responsible for first-aid care of students who are injured or become ill while in school.  Accidents of any kind should be reported promptly to the school Health Office.

Every student is required to have an Emergency Card on file at the school, with alternate contact information in the event that the parent/guardian cannot be reached in an emergency.  Emergency cards are provided to students at the beginning of the school year, and are also available online.

A physical examination is required by law for every new student entering school.  BCSD also requires a physical for all children entering pre-K, kindergarten, 2nd, 4th, 7th and 10th grades, as well as students in special placements.  Parents have the option of using a walk-in clinic, private or family physician, or the school's physician.  Medical Examination Forms must be returned to the student's school by Oct. 1st.  If the form is not returned, a school physcial will be scheduled.

A law recently enacted in NYS requires school districts to request a dental certificate from each student.  This Dental Certificate is available for you to take to your child's dentist and return the completed form to your child's school nurse or nurse-teacher.  The form will be filed as part of your child's Cumulative Health Record.  While we encourage you to do this, returning the Dental Certificate is optional.

Students who participate in interscholastic athletics must undergo a sports physical.  The sports physical is given in addition to any required school physical and MUST be conducted by the school physician.

Physicals for fall sports typically occur in spring and reviews are conducted in mid-August.  Winter & spring sports physicals are scheduled during the school year.  For more information on dates and times of physicals, call the School Health Office in the student's school.

Parents are also required to complete and return an updated medical history and athletic consent card allowing their child to participate in team sports.  A new card must be completed for EACH sport.  Cards are sent home with students and are also available in the Health Office.

State Education Law requires all students to participate in physical education.  Students who are unable to fully participate in physical education programs will have their programs modified to meet their needs.  A form is provided at the beginning of the school year for completion by a physician indicating what activities will be allowable for the student's participation.  This P.E. The district will provide homebound instruction to students who will be absent for an extended period of time when a written statement is submitted by a physician indicating the nature of the illness and the length of time homebound instruction will be needed.  The information shall be verified by the school physician.  Upon verification, the district will provide up to 5 hours of instruction weekly to K-5 graders and 10 hours of instruction to 6-12 graders.
